Behavioral Health Case Manager

Siskiyou Community Health is accepting applications for a full-time Behavioral Health Case Manager at our medical facility located in Grants Pass.

Position: Siskiyou Community Health Center has an immediate opening in our Grants Pass, Oregon clinic for a full time Behavioral Health Case Manager. The case manager functions as a core member of a collaborative care team that involves the patient’s primary care provider, a psychiatric consultant, and, when available, other mental health providers in the primary care clinic. The case manager is responsible for coordinating and supporting mental health care within the clinic and for coordinating referrals to clinically indicated services outside the clinic.

Essential Functions: Coordinate closely with patient’s primary care provider; screen and assess patients for common mental health and substance abuse disorders; provide patient education and monitor patients; support psychotropic medication management prescribed by PCP’s, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment; provide brief interventions using evidence based techniques; provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated or to other clinically indicated services outside the primary care clinic; participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s PCP; facilitate patient engagement and follow-up in care and track patient follow-up and clinical outcomes using a registry; document patient progress and complete relapse prevention plans with patients who are in remission.

Education and Experience: Candidate must have a high school diploma or equivalency; Bachelor of Science in Psychology or Social Work; and be a Registered Nurse.
