Substance Abuse Case Manager

Siskiyou Community Health Center is looking for a Substance Abuse Case Manager to join the dynamic team in our Grants Pass Primary Care Medical Center.

In this unique primary care setting under the direction of the Chief Medical Officer, the Substance Abuse Case Manager is responsible for counseling patients during the primary and structured recovery process while under their providers care. This individual will have a case load assigned to them as well as individual brief interventions in a primary care setting. This position requires the Case Manager to assist patients in recognizing that drug and alcoholism (substance abuse) is a disease, motivate them to actively participate in their own recovery process, assist them in learning skills and gaining knowledge and introduce them to the recovery process so they may live without drugs or alcohol.


  • Screen for substance abuse, provide brief intervention in a primary care practice and provide referral for treatment to community programs when indicated.
  • Work with Chief Medical Officer and Medication-Assisted Treatment (MAT) prescribing physicians to establish treatment protocols and workflows.
  • Provide screening for participation in Medication-Assisted Treatment (MAT) program in a primary care practice.
  • Provide case management for MAT patients, including communicating with community treatment agencies, insurance companies and other medical providers.
  • Provide patient education regarding MAT program participation.
  • Assist with medical appointments (rooming patients, checking and recording vital signs, screening tools/scales, collecting patient history, assisting with documentation in electronic medical record).
  • Assist with medication prior authorizations.
  • Knowledge of and adherence to confidentiality regulations (HIPPA and 42CFR regulations).
  • Knowledge of community treatment agencies and resources.
  • Conduct patient referrals to SCHC programs and other agencies.
  • Provide patient and community education on related topics including MAT and naloxone.
  • Active participation in additional committees and work groups as requested.
  • All other duties or special projects as assigned.


1. High School diploma or equivalent education preferred.
2. 1 year experience in drug and alcohol counseling preferred.
3. Medical Assistant or CNA experience helpful.
4. CADC certification preferred.
